1099 Notification Submission Form

We would like to offer our assistance with your member notification regarding your year-end substitute 1099-INTs (if you've opted to include these details on your statements).

By now, your credit union should've received a copy of the 2024 Year-End Processing Guide, which includes instructions for selecting your 1099-INT tax form handling preferences.

Please submit this form no later than 11/22/2024 to take advantage of our offer to assist with these member communications. There will be a one-time fee applied of $100 for any clients not currently using Xtend for HTML eStatement Notifications. We will not perform any maintenance or send any communications without first receiving your form submission. We look forward to hearing from you!
